Huntington disease (HD) is a very uncommon neurodegenerative condition of the central nervous system and it is characterised by dementia, irrational movements, a behavioural and mental health issues. In the Caucasian population, prevalence is thought to be between 1/10000 and 1/20000. 30 to 50 years old on average is when symptoms first appear. Juvenile Huntington's disease (JHD) symptoms often appear before the age of 20 and include behavioural issues and academic challenges. The typical symptom is chorea, which gradually affects all muscles. All psychomotor functions experience substantial retardation. Patients experience cognitive deterioration and psychological problems. HD is brought on by a long (36 or more repeats) CAG repeat on the short arm of chromosome 4p16.3, a hereditary autosomal dominant condition in the gene for Huntington. The earlier the illness develops, the longer the CAG repeat. Only healthy adult at-risk persons who want to know if they carry the gene or not should have pre-manifest diagnosis done by multidisciplinary teams. Other causes of chorea, such as general internal illnesses or iatrogenic disorders, are included in differential diagnosis. The CAG repeat increases in length as the disease progresses sooner. In JHD patients, the repetition rate typically exceeds 55. Only healthy adult at-risk individuals who are interested in learning whether they carry the gene or not should have multidisciplinary teams perform pre-manifest diagnostics. Other causes of chorea, such as general internal illnesses or iatrogenic conditions, are included in the differential diagnosis. A patient with difficulty in using left hand since last 2 days before admission to the hospital with k/c/o hypertension for 10 yr and history of numbness of left hand for past days. His blood investigations showed Vitamin D deficiency. MRI brain showed acute infarct in right frontoparietal, centrum semiovale, right post central gyrus, chronic infarct with gliosis in right premotor gyrus.
